Put foundation around the edges and blend as best as you can for now, it's not going to look perfect, yet. Then add some green and black to the actual tissue paper and that should take some attention off of the edges. Then take a light red and add to the cracks of the green and black.

Once all that is done and it is up to your standards drench your scar in fake blood and that sets it off to look gruesome, and very realistic.
